Our Palace Connectors programme gives people time to focus on what matters to them, taking a holistic approach to improve people’s health and wellbeing. Connecting people to community groups and statutory services.

Our trained community connectors take the time to get to know a person, giving them time to make sense of their situation and feelings, then jointly develop a plan to help the person access the right information, services and support.

Palace for Life have a range of staff trained in supporting family weight management and mental wellbeing and have experience working with those aged 4 – 90+, as well as dedicated youth workers and mentors that support breaking the cycle of youth violence. We also have health coaches who support those with long term health conditions to live more independently.

We deliver programmes, initiatives and activities in the below areas:

  • Employment, training and volunteering
  • Education and qualifications
  • A range of physical activity sessions and healthy lifestyle advice
  • Social support groups

How it works

Through our single point of access, a referral can be made from a range of local agencies, including GPs, hospital discharge teams, allied health professionals, police, job centres, social care service, housing associations and voluntary, community and social enterprise (VCSE) organisations. Our Community Connectors work with people on a 1-to-1 basis to tackle issues that may be exacerbating their health issues. Together, the individual and Community Connector identify local resources that align with the individual’s needs through a personalised plan. Through follow up support, the end goal is to help the individual build the knowledge and confidence to utilise resources in their community to improve their quality of life.
